Claims Process:
You
do need to file an insurance claim and you can get this
paperwork by either calling your customer service
number, which will be prominent on your fulfillment kit
or by calling the insurance company directly. This
number will also be listed in your fulfillment kit.
Claim Form
Note: You will need to call the hospital, insta-care, or
wherever you received service for your accident and
request a "UB-04" or a "HICFA 1500 itemized bill" for
the services rendered. This is the only form that they
will take. You will have to call the hosptial or insta-care
to request these specific forms be mailed to you. Once
you have received that bill, fax it in WITH the claim
form filled out to: 972-512-5820
