Below are the Hospital Indemnity Plans and summary of plans
offered in Utah. These plans work great with High Deductible Health
Plans (HDHP). Many people are turning to these HDHP's to bring down
their premiums. But it often leaves them with Thousands of dollars
to pay in the event of a major hospital stay. That is where many
turn to Hospital Indemnity or "Supplemental" plans.
We only offer these as examples and estimates only.

Cash Plan apx $60.00/month
(must choose a Critical Illness or other rider with the
Hospital Indemnity Plan) |
Payout of $2,000 initial hospital benefit with a $200 daily
hospital benefit / $800 ICU benefit. Prices depend on your
age and your situation. The package comes with your choice
of a Critical Illness benefit or a Children's life insurance
protection. Quotes are for ages 0-44. This plan can be
purchased to supplement your Group or Individual Family
Health Insurance Plan, and is HSA compatible.

Colonial
Life
- $90 / $75 / $35/month - Family / 2-Party / Single Rate |
Note:
You must apply for this option though a
payroll group. If you are self employed with employees you
can set up a payroll deduction plan as long as you still
have as little as 3 employees in the company and the premium
is paid through the payroll deduction from the company. Only
one employee needs to sign up for a plan as long as there
are at least 3 employees in the company. Examples below. See
agent for details.
Benefit is $3,000 for being admitted into a hospital for a
24 hour period. No further benefit is given regardless of
length of stay.
So $3,000 for
a 2 day stay, or $3,000 for
a 4 day Stay. This policy is HSA Compatible. There are other
options that can be customized for your needs - some HSA
compatible and others not. There is also a disability plan
that can pay the Employee for an extended absence from work
after a hospitalization when the Employee is not able to
return to work for at least a week, etc.
Example A: You are self employed (or a spouse
is) and you have no employees or only one other employee.
This option will not work for you unless you can get a group
situation like Example B below.
Example B: You are self employed (or a spouse is) and
you have at least 2 other employees (one of which can be the
spouse) - Or you are an employee of a group that has at
least 3 employees (including the owner). We set up a payroll
group benefit at your place of work. Only one policy needs
to be submitted at the time the group is set up, but there
needs to be at least 3 employees in the company to start the
benefit.
Monthly Premium Summary for Example B:

Benefit for Example B:
Employee (Spouse
or family member if that option is chosen) is hospitalized
for at least 24 hours. is hospitalized for covered surgery,
sickness, accident, maternity, etc. They would receive
$3,000 for this covered hospitalization. Note:
Pre-existing conditions would NOT be covered if the
condition that caused the hospitalization was existing at
the time of application. (ie you have a back surgery
scheduled, or you are pregnant, etc.)

Common Questions and
Answers:
Q) Why can't I go directly through the companies instead of an
Agent like you?
A) The companies
do not have a marketing department. They rely on Agents like us to
take your phone calls, answer you questions and process your
applications. If you tried to call the companies directly they would
send you to one of us to take care of you. You don't pay us, the
companies compensate us by paying a commission based on the number
of applications we send in.
Q) When will my policies
be in force (ie What will be the Effective Date)?
A) Option #1
will start within a few business days of when they get the
paperwork. You can also ask for a future date. Option #2 will start
within a few business days of when you apply. Option #3 can be
effective on your signature date.
Q) Can
anyone give me a lower price?
A) No. We all
work off the same rate sheets so you can't get a "sweetheart" deal
by shopping around. I really wish I could get a lower price, but it
doesn't work that way. We do more supplemental maternity plans like
this than any other Agency in Utah. Some new Agents try the business
but don't get enough volume to maintain their contracts with the
company. We have been doing this since 1997 and will be there for
you in the future to answer your questions, etc.
Q) Do
I pay you, or the company?
A) You pay all
money to the company. We collect nothing but a VOID check to send to
the company to set up payments. Both of the companies will take a
credit card payment also. Remember that you are contracted with the
company, not me.
Q) When will the first withdrawal happen?
A) Generally the money will come out within the next week or so for
the first month's premium.
Q) Can I cancel these plans anytime I want to?
A) We of course encourage you to "build these plans into your long
term insurance portfolio. But if you decide to cancel be very
careful to make sure the policy continues to be in force through any
claims you may have. But yes, you can cancel anytime month to
month. There is no obligation on your part to continue any longer
than you would like to continue.
Q) What will I get from the companies to to give evidence of
my policies?
A) You should receive paperwork from each of the companies within a
week or two of your applications.
Q) How would I file a claim?
A) If you or a covered family member has a claim (hospitalized
for at least 24 hours) you can fill out a claim form and fax that
claim form to the company with a copy of your hospital bill. See

Q) Can I pay by Credit Card?
A) Options #1 allows credit card payments and does not
charge extra for Credit Card payments. Option #2 & 3 do not
accept credit card payments - they must be withdrawn from a bank
account each month.
Q) Are there any start up fees, Application Fees or other
charges?
A) Yes. Option #2 has a one time non-refundable $20
application fee. None of the others have a fee associated with it.
